Beginning a Residency
One year of post-doctoral supervised work experience is required for licensure as a psychologist.  Three years of post-masters supervised work experience is required for licensure as a psychologist associate.
 
You may submit your proposed residency contract before your application is approved, however the a residency contract cannot be approved until your application is approved (i.e. when you are a candidate for licensure).

FORMS:
 
Psychologist Resident Supervision Contract- this form should be completed by the resident and supervisor and submitted to the Board's office.  It must be approved by the Board before beginning post-doctoral supervised work experience.
 
Psychologist Associate Resident Supervision Contract- this form should be completed by the resident and supervisor and submitted to the Board's office.  It must be approved by the Board before beginning post-masters supervised work experience.
 
Record of Supervised Hours (i.e. the Hours Log)- this form is used to keep track of supervised work hours as you go, and is submitted at the end of the residency.

Ending a Residency
Final Evaluation of Resident Form- to be completed by the primary and associate supervisor (together, if applicable) at the end of the residency, and submitted to the Board's office.
 
Final Evaluation of Supervisor Form- to be completed by the resident at the end of the residency.  Please send in attestation slip only.
